An Aggressive and Safe Approach for Dyslipidemia Drug Therapy*

چکیده

The National Cholesterol Education Program (NCEP) Third Adult Treatment Panel (ATP III) guidelines call for immediate intervention in individuals with established coronary heart disease (CHD). The significance of immediate intervention was based in part on our understanding of plaque stability versus morphology (ie, the nature of the plaque, not the size, determines its vulnerability to thromboses). The sequelae of a ruptured plaque are dangerous and often deadly. Statins, through their ability to significantly reduce lowdensity lipoprotein cholesterol levels, can drastically reduce the risk of a future CHD event. This article reviews the epidemiologic statistics regarding CHD, with an emphasis on the lethality of this disease and the overall risk versus the risk of any side effects of statins. The risk factors for CHD as outlined by the NCEP ATP III guidelines are reviewed. The guidelines recommend 2 types of treatment based on CHD risk: therapeutic lifestyle changes and drug treatment. Realistic outcomes with these types of therapies are reviewed as well as ongoing clinical studies and new drug therapies recently approved by the US Food and Drug Administration. Aggressive and early intervention to prevent CHD (both primary and secondary prevention) is essential, and it is quite safe. The onus is on primary care physicians to identify patients early and initiate statin therapy before the first clinical signs of CHD develop. (Adv Stud Med. 2004;4(1B):S44-S52)

Combination Therapy versus Monotherapy for Dyslipidemia : Are 2 Pills Better than 1 ?

Accumulating clinical trial evidence and recently updated national guidelines support more aggressive efforts to reduce levels of circulating atherogenic lipoproteins. To meet the new, intensified low-density lipoprotein cholesterol goals issued by the Third Adult Treatment Panel of the National Cholesterol Education Program, dyslipidemic patients at high cardiovascular risk may require reducti...

An Evidence-Based Approach to the Use of Combination Drug Therapy for Mixed Dyslipidemia

Dyslipidemia is one of the major modifiable risk factors for coronary heart disease (CHD), the leading killer of men and women in the United States [1]. Most patients with CHD have multiple lipid abnormalities, and a management approach focusing exclusively on lowdensity lipoprotein cholesterol (LDL-C) reduction fails to address the added atherogenic potential that has been shown to exist when ...

An overview of glucagon-like peptide-1 receptor agonists for the treatment of metabolic syndrome: A drug repositioning

Metabolic syndrome (MetS) is a clustering of several cardiovascular risk factors that include: obesity, dyslipidemia, hypertension and high blood glucose, and often requires multidrug pharmacological interventions. The management of MetS therefore requires high healthcare cost, and can result in poor drug treatment compliance. Hence drug therapies that have pleiotropic beneficial effects may be...

Managing diabetic dyslipidemia: aggressive approach.

The United States is experiencing a marked increase in rates of diabetes mellitus and metabolic syndrome, almost certainly in part due to the increase in obesity rates. This phenomenon is likely to also result in an increased risk of coronary artery disease as risk factors increase exponentially. This article defines diabetic dyslipidemia, the rationale for aggressive treatment, and options for...
